As a user of the Anamatix© World Wide Web site (the “Service”) your privacy matters to us. You are entitled to know how your use of the Service will affect your personal privacy. The purpose of this Privacy Policy Statement is to inform you about:
- what information about you will be collected by Anamatix, Inc. ("Anamatix"), the operator of the Service,and the circumstances under which the information will be collected;
- how the information will be used or disclosed to others;
- what choices are available to you regarding collection, use and distribution of the information;
- what kind of security procedures are in place to protect against the loss, misuse or unauthorized alteration of information under Anamatix’s control; and
- what steps you can take to have inaccuracies in the information corrected.
Disclosure
In the normal course of operations it is likely that we will maintain personal information on computers in the United States and Canada.
Application
This Privacy Policy Statement applies only to the Service and not to Web sites operated and maintained by other companies or organizations to which the Service is linked, or companies with whom you may engage in transactions for the sale of products or services through the Service. Children under 18 years of age are forbidden to use the Service and Anamatix does not knowingly solicit or collect information about children under 18 years of age.
Information Collected By Anamatix
The types of information that Anamatix may collect about you, either as a user of the Service or as a participant in the Anamatix Interactive Fitness Network ("Network") through a physical therapy facility, employer, payer or any partner website ("Fitness Facility"), are limited to the following:
Volunteered Information:
This category consists of (i) information that you voluntarily provide to Anamatix upon your registration to use the Service, such as your name, email address, and other contact information, (ii) information that you subsequently volunteer to Anamatix regarding your health information, assessments and exercise plans through the Password protected features of the Service, or changes in your registration information, (iii) if you are a participant in the Network through a Fitness Facility, the information that you provide to the Fitness Facility at the time of enrollment and thereafter through the Network, either by you directly inputting the information into the Network or by using training equipment that automatically inputs the information into the Network, and (iv) other information about you that you may voluntarily provide through the Service at Anamatix’s request.
Transaction Information:
This category consists of information you may provide to Anamatix as a result of your purchase of goods or services from the Service and utilization of the Service, which may include exercises performed, clinical provider, employer, insurance company, contact information, billing and shipping addresses and financial information such as credit card number and expiration date. Information that you may provide to a company other than Anamatix during a transaction consummated through the service is not included in this Privacy Policy Statement and will be subject to the privacy policies and practices, if any, employed by the company with whom you are dealing.
"Cookie" Information:
As is the case with many Web sites, the Service uses standard technology called "cookies," which are small data files that are downloaded onto your computer when you visit a particular Web site. Cookies automatically identify your browser to the Anamatix server whenever you interact with the Service. Anamatix uses cookies to track the pages you have visited within the Service, but cannot track any activity by you on other Web sites. If you prefer not to have any information collected by cookies, you may disable cookies by turning them off in your browser; however, some areas of the Service may not function properly if you do.
All of these types of information about you may be maintained by Anamatix on computers located in the United States and in the United Kingdom, and may be transferred from time to time among those computers.
1. How Information Will Be Used or Shared With Others.
Except as provided in this Section, Anamatix will not use or disclose to any third party any personally identifiable information about you for any purpose without your express prior consent.
Volunteered Information:
Volunteered Information will be used by Anamatix to operate the Service and maintain your user account within the Service, including the record of your exercises and other health and fitness information provided through either the Service or the Network. For this purpose it will be necessary to disclose Volunteered Information to Anamatix employees and contractors who have a need to know.
Volunteered Information relating to your health and fitness assessments and performance and training will be used by Anamatix to combine it with information of other users of the Service to develop aggregate statistical information relating to issues of fitness, exercise, athletics and health. Aggregate statistical information may be used by Anamatix or disclosed to third parties, but will not contain any personally identifiable information about you.
If you are a participant through an employer, insurance, health or wellness organization or health care provider Information relating to your health and fitness performance, assessments and training will be made available to you and to your respective employer, insurance company and or health care provider.
Volunteered Information will be used by Anamatix to send you information about Anamatix and promotional material about products and services of Anamatix and other companies that we think may be of interest to you, unless you elect not to receive such information.
Transaction Information:
Transaction Information will be used by Anamatix solely for the purpose of completing the transaction you have requested and providing customer service and with respect to that transaction. Cookie Information: Cookie Information (which is identified to your browser, and not to you personally) will be used only to assist in your navigation within the Service, to monitor consumer interest in the range of products and services available on the Service, to assist Anamatix in tailoring the content of the Service to users’ needs by collecting information about their preferences through tracking patterns of page-viewing on the Service, and to assist Anamatix in delivering content that appears to meet your preferences. Disclosures Required By Law: Anamatix may be required by law or legal process to disclose information about you in its possession.
2. What Kind of Security Protects Your Information.
The first time that you use the Network you will be given a pin number that must be used to transmit your health and fitness performance and training information to Anamatix. When you become a registered user of the Service, you select a personal User Name and Password that must be used to gain access to your account on the Service. Your PIN number (for access to the Network) and your User Name and Password (for access to your Service account) are important elements of keeping your information private and secure from misuse or unauthorized alteration. You should not disclose them to anyone.
3. What Steps Can You Take to Correct Information.
Anamatix will promptly correct any errors in any information it has about you upon your written request for such correction. To request a correction, contact Anamatix at either of the contact points described in Section 5. The requested correction will be made as soon as practicable after receipt of the notice by Anamatix.
4. Modifications to Privacy Policy Statement.
Anamatix may modify this Privacy Policy Statement at any time, at its discretion, and modifications are effective upon being posted here on the Service. You are responsible for revisiting this Privacy Policy Statement periodically to ensure that you are aware of any changes to it.
