Dr. William Maloney, M.D.

Dr. Maloney, Chair, Orthopaedic Surgery Professor at Stanford University Medical Center, is recognized nationally and internationally for his contributions to the improved understanding of the causes of failure of surgical joint replacement. His research in the area of joint replacement has twice won awards from the Hip Society. Dr. Maloney is a member of the Central Program Committee of the American Academy of Orthopedic Surgeons, and he chairs multiple additional national committees, including the National Hip and Knee Registry Work Group of the American Academy of Orthopedic Surgeons, the Awards Committee of the Association of Bone and Joint Surgeons, the Awards and Publications Committee of the Hip Society, and the ABC Exchange Fellowship Subcommittee of the American Orthopedic Association. He has been a Visiting Professor to numerous universities and institutions throughout the United States and Asia. Dr. Maloney has a Board Certification in Orthopedic Surgery, from the American Board of Orthopaedic Surgery.

Dr. Wendy Graham, M.D., CCFP, FCFP

Dr. Wendy Graham, the Founder of AFHTO, The Association of Family Health Teams, is the past Medical Director and Lead Physician with North Bay’s Blue Sky Family Health Team and has been in full-time comprehensive family practice for over 20 years. Dr. Graham was recently described as an “influential physician“ in primary care reform and collaborative care models for Canada. Presently, she is a member of the CEO round table for the Local Integrated Health Network 13, and contributed to the Aging at Home Strategy Committee. She has been awarded the College of Family Physicians of Canada, Dr. Jean-Pierre Despins Award for 2004 and the Distinguished Service Award from the Ontario Medical Association for 2002. She is a past member of the Institute of Optimizing Patient Outcomes Board, Canadian Council for Integrated Health Care, and Ontario Medical Association Board of Directors. Presently Dr. Graham holds an Assistant Professorship at the University of Ottawa and acts as a preceptor for Northern Ontario Rural Medical School.

Dr. Robert S. Millard, M.D.

Dr. Millard graduated from college at Michigan State University and attended medical school at Wayne State University in Detroit, MI. Dr. Millard began his private practice at Sports Orthopedic and Rehabilitation (SOAR) in 1993. He is Board-Certified in Physical Medicine and Rehabilitation, and has presented and authored on many topics within the areas of non surgical management and rehabilitation of musculoskeletal injuries. Dr. Millard has been team physician of several college and professional organizations including San Francisco 49ers, San Jose Sharks and Santa Clara University.

Dr. Gary S. Fanton, M.D.

Dr. Gary Fanton is the Chief of the Section of Sports Medicine at the Stanford University Department of Orthopedic Surgery. His practice primarily involves the diagnostic evaluation and treatment of sports and trauma-related injuries of the upper extremity, knee, and ankle. He is Board-Certified by the American Board of Orthopedic Surgery. He has been in practice in the Bay Area for over 22 years and his experience includes orthopedic consultant for the San Francisco 49ers, team orthopedist for the San Francisco Giants and Stanford University, company physician for the San Jose Ballet, head team orthopedist for Stanford’s additional 29 varsity sports. He co-founded the SOAR clinic, where he was an active partner in private practice since 1983. He holds a BS from the University of Michigan and an MD from the Medical College of Wisconsin.

Jeff Ferguson

Jeff Ferguson is the Head Athletic Trainer for the San Francisco 49ers. Mr. Ferguson oversees the team’s rehabilitation procedures, injury records, training room and daily athletic training duties. Mr. Ferguson joined the 49ers organization after serving as the manager of athletic team development with BREG, an orthopedic company based in Vista, CA. Prior to his move to the West coast, Ferguson spent eight years in several capacities at Kansas State University, where he was responsible for the prevention, evaluation, treatment and rehabilitation of all athletic injuries; he also supervised medical coverage for 14 collegiate athletic teams. Mr. Ferguson earned his bachelor’s degree in 1993 at Clemson University and his master’s degree in 1995 at Texas-Arlington. He is also a certified member of the National Athletic Trainers Association (NATA).

Olaf Evjenth

Mr. Evjenth is a Norwegian physical therapist who is world renowned in the field of manual therapy and has contributed expertise to the development of the Anamatix assessment programs. He also assisted in the development of the biomechanically-based Kaltenborn-Evjenth System of Manual Therapy and has taught throughout the world for 50 years. He has authored texts on manipulation, physical training, stretching and diagnostics in manual therapy. Mr. Evjenth was a champion athlete and the physical therapist for many Norwegian National and Olympic teams.

Ann Hawkins

Dr. Hawkins is a noted expert in sports management, well care and fitness with specialization in corporate/hospital/well care relationships and developments. She developed WellCare Dimensions, Inc. in 1994, a company that is focused on the integration of exercise and education within the health care/fitness marketplace. Dr. Hawkins is on the Board of Directors of 24 Hour Fitness, one of the largest fitness center chains in the United States. Her doctoral thesis was presented on the financial benefits of well care.
